Pre-vocational Services and Life Skills Development for young adults with disabilities ages 21-25+

Inroads TransitionPLUS serves individuals with disabilities between the ages of 21-25+ that are interested in daily living skills and/or pre-vocational training by providing consistent and structured professional support to help them navigate the challenges that often arise due to their disabilities.

Our TransitionPLUS programs give young adults a place to access assessments, develop independent living skills, explore career possibilities and resume boosting opportunities, train for self-advocacy, enjoy volunteer, community integration, and leisure experiences, benefit from case management, build social skills, friendships and network with peers.

TransitionPLUS offers two tracks.

TransitionPLUS “Foundations” includes: 24 Units of Day Habilitation

  • Communication and self-advocacy skills

  • Volunteer opportunities

  • Evaluation of vocational and leisure interests

  • Identification and exploration of current and future goals

  • Exploration of recreational and leisure interests

  • Community integration experiences

  • Independent living skills development

  • Interpersonal and relationship building skills

TransitionPLUS “Work-Ready” includes: 12 Units of Pre-Voc per Day + 12 Units of Day-Hab

  • Vocational skills for employment readiness

  • Communication and self-advocacy skills

  • Evaluation and exploration of work readiness skills

  • Employment options and career planning goals

  • Community integration and networking skills

  • Resume boosting work and volunteer opportunities

  • Independent living skills development

  • Interpersonal and relationship building skills

Young adults may be eligible for this program through the NJ Division of Developmental Disabilities.
